Biographical Notes:

Flying a Kittyhawk from RNZAF Ohakea on a formation exercise, Douglas Ayson was seen to leave the formation and execute several diving turns before crashing into Otaki beach. It is believed that the pilot may have been overcome with fumes from a leaking fuel system.
