Biographical Notes:

Thomas' service record shows that he was killed while carrying "Mills Bombs" (hand grenades) in a sandbag. A court of inquiry into the accident suggested that no-one was to blame, but the detenators should have been removed, as the grenades probably exploded due to the safety pins breaking.
